If a married woman wishes to change her name or surname in Passport after marriage then at the time of appointment she must be present with their passport application and the Photostat copy of the passport of the husband if available at the Passport Seva Kendra or Regional Passport Office. Otherwise, a self-attested photocopy of the Marriage certificate issued by Registrar or a joint affidavit of Husband and Wife with Joint Photograph sworn before the Notary on a Non-Judicial Stamp Paper of minimum value will also do.

How to change the name in passport after re-marriage?

If an applicant wishes to change his/her name or surname or both in Passport after re-marriage then he/she must be present with his/her passport application at the time of appointment at Passport Seva Kendra or Regional Passport Office along with the divorce deed or death certificate with respect to the previous spouse or a self attested photocopy of the Marriage certificate issued by Registrar or a joint affidavit of Husband and Wife with Joint Photograph sworn before the Notary on a Non-Judicial Stamp Paper of minimum value will also do.

How to change the name in passport under other circumstances?

If an applicant wishes to change their name or surname or both in passport under any circumstances then he/she must be present with the following document with their passport application at the time of appointment at Passport Seva Kendra or Passport Regional Office.

  • Deed poll/Sworn Affidavit by a Notary on a Non-Judicial Stamp Paper of a minimum value.
  • Newspaper Advertisement in any two daily leading newspapers in Original.(The newspaper should be published the city of Present Address of the Applicant.)
