Employees Provident Fund (EPF) is maintained by the Employees Provident Fund Organization, India. For the salaried employees, employees provident fund is a most valuable investment. EPF is compulsory for the employees with the basic salary up to Rs 6500. And, the contribution for EPF is optional for the employees whose basic salary is above Rs 6500/-. After getting a minimum service of seven years, any salary based individual can avail various advantages.

An employee can withdraw an amount from EPF account in advance for their children’s education, marriage, construction /purchase of a house, medical emergencies, etc. A maximum of 3 withdrawals, one can make from EPF Account, and the maximum amount will be 50% of your EPF contributions.

Form No: Statement IW-1

Form No: 5(IF):

This form is preferable for the person qualified to receive the Employees Provident Fund dues of the departed member who died during services.

Eligibility:

  • Members of the family (Nominees) proposed under EPF Scheme.
  • If there are no nominees then, all the family members can take benefit of EPF (Except major son & married daughters whose husband is alive).
  • If there is no family & nominees, in that case, a legal successor is eligible.
  • If both the parents are lost in that case Guardian of a minor Nominee/Family member/Legal successor.

Documents Required:

For Spouse:

  1. Death Certificate of a member.
  2. A copy of a cancelled/blank cheque of the account in which payment opts.
  3. In case the members were last employed under an establishment exempted under the EPF Scheme 1952, the employee of such institution should specify the PF details of last 12 months under the Certificate part and also provide an attested copy of the Member’s Nomination Form.

For Guardian on behalf of Minor, Nominee & Legal Heir:

  • Death Certificate of the member
  • Guardianship certificate
  • Succession certificate
  • A copy of a cancelled/blank cheque of the account in which payment opts.
  • In case the members were last employed under an establishment exempted under the EPF Scheme 1952, the employee of such institution should give the PF details of last 12 months under the Certificate part and also furnish an attested copy of the Member’s Nomination Form.

Requirements for filing the Form no: 5(IF):

  1. EPF Account Number
  2. Details of Bank Account for receiving payment
  3. Details should be entered in BLOCK LETTERS

Note:

  1. For SMS alerts, write your mobile number on the top of the form.
  2. Along with Form no: 5(IF) to receive all the benefits of claiming, submit – the Provident Fund (Form 10 D), for the pension (Form 10 C) & withdrawal form.
  3. The employer should attest the application, where the member was last employed.
  4. If the firm is shut or closed, then the application form may be certified by any of the officials.

Application Form Submission – List of Officials:

  • A Magistrate
  • A Gazetted Officer
  • Post/Sub-Post Master
  • President of the Village Panchayat where there is not Union Board,
  • Chairman/Secretary/Member of Municipal/District Local Board,
  • Member of Parliament/Legislative Assembly
  • Member of CBT/Regional Committee EPF
  • Manager of the Bank in which the Bank Account is maintained
  • Head of any recognised educational institution.

Form No: 10-C (Employees Pension Scheme):

Eligibility:

  • Personnel whose service rendered from 1.3.1971 to 15.11.1995.
  • Personnel who left the employment before completion of 10 years service period.
  • An employee who had attained the age of 58 years before completion of 10 years service whether in service or left the service.
  • Employees who have completed 10 years service on the date of leaving, and has not attained the age of 50 years at the time of filing this application.
  • Employees who reached the age of 50 years or more, but less than 58 years and not willing for reduced pension.

Documents Required:

  • A copy of blank/cancelled cheque.
  • Date of Birth Certificates of children of the member, if applying for Scheme Certificate.
  • Death Certificate of a member in case of mortality of member specified in the Scheme.
  • Succession Certificate in the event of an application by the legal heir after the demise of a member.
  • Affix Rs 1 stamp if opted for payment of withdrawal benefit through Bank.

Documents Required:

  • Claim Form
  • 3 passport size photographs if requested by the member Joint photo with the spouse.
  • If claimed by widow/widower the photo should be sent for widow/widower and his/her two children (below 25 years) separately.
  • The employer or his authorised official, indicating the person, whom the photograph relates, should attest the photos.
  • Account No. of the member, written in verse and placed in a separate envelope.
  • The application should be forwarded to the company in which the member last served/died.
  • The institution or company should furnish the certificate and wage particulars duly attested by the authorised officer.
  • If the institution or company is closed, the application should be forwarded through Magistrate/Gazetted Officer/Bank Manager/any other authorised officer for the approval by the Commissioner.

Form No: 13- helpful in transferring EPF Account when an employee changes from one company to another.

Form No: 19

Application Form no 19 is helpful in withdrawing his/her EPF funds on leaving service/retirement/termination.

Eligibility:

  • Attained and retired from service after reaching the age of 55 years.
  • Retired on account of permanent and total incapacity for work due to Physical/Mental infirmity.
  • Retired under voluntary retirement scheme.
  • Migrating from India for permanent abroad settlement/taking up employment abroad.
  • Retrenched from service.
  • Discharged from the service of receiving compensation, under the Industrial Dispute Act, 1947.

Documents Required:

  • A copy of the blank/cancelled cheque.
  • A medical certificate from the ESI.
  • If the member retired in aspect of permanent and total incapacity – submit the reason for leaving the job.
  • In the case of migrating from India for permanent abroad settlement– A copy of Visa, Passport Journey Ticket.
  • The offer of appointment letter and Copy of Visa, Passport Journey Ticket – In the case of taking up employment abroad.

Form No: 20

Application Form 20 is beneficial in claiming the EPF funds by a nominee in the event of the demise of a member.

Eligibility:

  • Members of the family (Nominees) proposed under the EPF Scheme.
  • If there is no family & candidates, in that case, a legal successor is eligible.
  • If the nominee is minor, then there should be representative as his/her guardian.

Documents Required:

  • Death Certificate of the member
  • Guardianship certificate
  • Succession certificate
  • A copy of a cancelled/blank cheque of the bank account in which payment opts.
  • Form 5(IF) for claiming the benefits under the Employees’ Deposit Linked Insurance Scheme.
  • Form 10D for claiming the Pension Benefits.
  • Form 10C for Withdrawal Benefit.
